All families are required to provide a signed copy of this Return To Play Protocol Acknowledgement and Waiver for each player.
No player will be allowed to return to play prior to receipt of a signed acknowledgement. Your signature is your agreement to follow the established terms and Return to Play protocols. Also, you understand that failure to abide by the procedures may result in removal from play. A copy of the signed Return To Play Protocol Acknowledgement and Waiver should be brought to the first scheduled session and given to your head coach.
