I am aware and understand that I am receiving a Goldwell hair colour to apply myself under a professional colourists instructions and that in some individuals, this can cause an allergic reaction.

A strand/ patch test of the same colour has been performed in salon prior to this chemical application. Chemical services are based on the individuals hair. I have made the lush colourist aware of any chemical changes I have made to my hair since my last lush colour or of any changes I have made to my hair routine prior to the delivery of this product. There are products recommended and available to me to help improve the integrity of my hair and to maintain the results after my hair colour.

I acknowledge that I am purchasing this product with the full intent to apply the hair colour myself with the understanding I have strict instructions to follow and that this agreement is only valid throughout the salon lockdown period in affect due to the COVID-19 virus across our nation and that this product will only be supplied under the legal terms of trade according to the current restrictions surrounding COVID-19.

I acknowledge that the colour provided must be applied on the same day of delivery or salon pick up and that the recommended timing of between 35-45 mins that is advised to me in consultation is to be strictly adhered to in order to achieve my desired result.

I will not hold liable The lush Collective hair + skin North Ryde in the unlikely event the process has unexpected or undesired results or reaction.
