By signing below, I acknowledge and agree to the following:

I authorize Alo Esthetix to apply eyelash extensions to myself on today's date and any future eyelash extension services.

I understand that because of the natural lash cycle and wear and tear, I will need to maintain my extensions with refill appointments usually recommended about every 2-3 weeks in order to keep them looking their best and for the health of your lashes.

I understand that I am to avoid all eye makeup and getting the lashes wet for 48 hours after my service. I understand to keep my eyes closed throughout the process as tearing can cause the lashes to bond or clump together rather that one on one.

I understand I have 48 hours to contact Alo Esthetix if I have lost an unreasonable amount of lashes or for whatever reason am unhappy with them. We will gladly touch up and replace any lost lashes within those 48 hours at no charge. After 48 hours you will be charged accordingly.

I hereby release any and all persons representing this salon from all claims, demands, damages, actions and cause of action arising out of the performance of the service.
