Disclosure:

Eyelash Extensions are Individual lashes, made of synthetic fiber such as nylon and or silk or real mink fur which are attached with medical grade glue on top of your natural eyelashes. Eyelash extensions last up to 3 weeks if after care instructions are followed properly. Fills are required every 2-3 weeks and will only be considered fills if 40% of your lashes are still attached. Eyelash extensions will differ in look and retention from person to person depending on the individual’s natural lash count, lifestyle, adherence to after care instructions, weather, moisture, and natural lash shedding cycle. No guarantees are expressed or implied.

Risks and potential complications include but are not limited to redness, irritation and allergic reactions to either adhesive and or eye pads, and loss or damage of natural lashes. To minimize the risk of eye injury, it is your responsibility to lie completely still for the entire procedure. Do not attempt to remove the extensions on your own.

Lash Removals:

A potent lash removal cream is applied to the base of the lashes and left for 15-30 minutes. There is a small risk of the lotion traveling into the eyes which may cause discomfort and a stinging sensation. If you feel anything as such please notify your lash artist and we will wipe it off immediately. This action will subside the discomfort and if eye flushing is needed your artist will do so.

After Care Instructions:

Wash lashses daily starting immediately after service
Avoid Oil based products
Brush lashes daily
Use setting spray to hold lashes in place in between fills

Consent:

I authorize any eyelash tech from OMNIA STUDIO to provide and apply eyelash extensions. I certify that I understand the risks and potential complications involved in getting this procedure and that I knowingly and voluntarily consent to the application of eyelash extensions. I understand that it is my responsibility to let my lash tech know if at any moment during the service I am uncomfortable and wish to end the service. I acknowledge that no guarantees or promises regarding the appearance or retention of the lash extensions have been made.
