RISK OF PHYSICAL INJURY

I understand that all activities with Dance Discovery have a risk of physical injury.

SUPERVISION

I understand that Dance Discovery teachers cannot supervise children in online classes and that i am responsible for my child’s supervision. Dance Discovery is not a licensed childcare facility.


SPACE

I understand that it is my responsibility to take every precaution to ensure that my child’s dance space is safe from hazards that could cause injury and that Dance Discovery teachers cannot control my child’s online class environment.

HEALTH RELEASE

I have had my child examined by a doctor and my child has been deemed healthy enough to Participate in online movement classes.

Photography – Video

Dance Discovery has my permission to photograph and videotape my child during online classes and performances. I agree that these images of my child may be used for ongoing public advertisements and that i will not revoke this permission.

ARTISTIC PROPERTY

I understand that Dance Discovery owns all curriculum delivered in online classes and that it may not be shared or duplicated without written permission.

THIRD PARTY INFORMATION

 I understand that Dance Discovery must share my email address with Zoom in order for me to receive online classes.

RELEASE of LIABILITY- I agree to release and hold harmless Michelle Denny, dba Avery Ranch Dance & Music, and Dance Discovery of Austin LLC, its instructors and assistants from any liabilities including accidents, injuries or other occurrences that may be sustained by the participants and/or the undersigned, while participating in online classes. I understand that with any physical activity injury may occur.
