
Repair Release Form
The undersigned (“Releaser”) requests services from For My Cell, LLC to repair a broken phone, tablet or other electronic device (“Device”). Releaser understands and agrees that this repair is an attempt to restore usable functionality via a singular repair. Releaser understands this is a risky repair and that many things can render the device unusable including, but not limited to, such things as damage related to motherboard prior to attempted repair that was not detected prior to repair, static discharges, and reported issues and releases For My Cell, LLC from any liability arising from the attempt.
The releaser understands when repairing a water damaged device there is no guarantee of the longevity of the device from there on and For My Cell, LLC will not accept any refunds of parts upon leaving the store with the device. Replacement parts from water damage repair will not be covered under the For My Cell, LLC warranty policy. Releaser further understands that this will void any warranty currently held by the device and understands that the labor fee will not be refunded in any case.
By signing below, Releaser forever releases and discharges For My Cell, LLC, it's employee’s and related entities from any manner of claims, demands, liabilities or obligations of any nature resulting from, or claiming to result from, in whole or in part, the work done by For My Cell, LLC on the device provided to For My Cell, LLC by Releaser.
***iPhone 7/8 series LCD/Digitizer repair may cause the fingerprint sensor/home button to become inoperable.
For My Cell, LLC Will not be held responsible for this malfunction.
***iPhone 6/6+ may have underlying touch IC Chip issues. This can cause a loss of total touch on all screens.
Releaser understands that For My Cell, LLC will not browse through any personal, private or confidential information or data; however, technicians may inadvertently see data during the course of the repair. For My Cell, LLC strongly suggests providing or deactivating the password or pattern for the best results of repair. Providing the passcode lets our technicians conduct tests before and after the repair to ensure the functionality of the device.
