PLEASE READ AND UNDERSTAND THE FOLLOWING:

​-I understand there are risks associated with having an eyebrow lift and/or eyebrow tint. I further understand that as part of the procedure, local irritation, local itching, local discomfort, and in rare cases allergic reaction could occur. You may have a patch test at least 24 hours before your appointment if you wish. I agree that if at any time I am uncomfortable with the eyebrow lift and/or eyebrow tint treatment, I will inform the technician and she will gladly rectify the problem, including ending the session.​-I understand that I cannot wet my eyebrows for 48 hours after the treatment.
-There are no guarantees for the length of time your eyebrow lift and/or eyebrow tint will last. I understand the aftercare instructions and will do my part to maintain my eyebrows. I understand that there are many factors that may affect the life of the eyebrow lift and/or eyebrow tint such as water and moisture contact, weather conditions, and activities involving exposure to high temperatures.
-We strongly suggest not getting this treatment if you are PREGNANT, nursing, have trichotillomania, or have recently undergone chemotherapy. The longer and thicker your natural eyebrows are, the better the results will be. We can do this treatment on thin brows, but if you do not have many eyebrow hairs, we suggest not getting this treatment done. This treatment can be done over micro landing/tattooed brows.


The technician will not refund or credit any amount of money because of a clients unhappiness with their final results.

I take sole responsibility for any reaction I may have, staining of clothing and/or personal belongings. I agree to hold Pampered & Polished and all authorized representatives harmless from any liability involved in the eyebrow lift and/or eyebrow tint process. Pampered & Polished and their staff  has explained this procedure to me and all my questions, if any, were answered. I have reviewed and completely understand all the information.
