Waiver: I/We as parents or guardians of the above named participant, hereby grant permission for him to participate in the MGBasketball camps/ Hoops Elite team activities and acknowledge the fact that he/ she is physically able to participate in all activities. I/We hereby release MGBasketball and Hoops Elite and its employees from all claims for illnesses and/or injuries which may be sustained by our participant. Furthermore, I/We authorize the director or designee to select hospital facilities and/or physician of choice and authorize treatment of the above named participant on an emergency basis in the event such treatment becomes necessary while attending MGBasketball camps or Hoops Elite activities. MGBasketball and Hoops Elite will not be responsible for loss or theft of money or personal articles. I/We hereby forever release MGBasketball Instruction, Hoops Elite, its employees, coaches and owners from all liability for any and all damages and injuries suffered by my child while under the instruction, supervision, or control of any of the above so mentioned.  I/We hereby forever release all owners, employees, or designated supervisors of any facility in use for the above mentioned activities from all liability for any and all damages and injuries suffered by my participant while under the instruction, supervision or control of MGBasketball or Hoops Elite.
