I, name as listed below, understand and agree to the following terms and conditions for receiving a Quarantine Color Kit from Corinne's As You Like It Salon.

• ​I release and discharge Corinne's As You Like It Salon, It's officers, directors, shareholders, and employees from any and all liability for personal injuries to myself or others, or any other claims including claims that are known or unknown, foreseen or unforeseen, past, future, or contingent third party claims arising from or in connection with the Quarantine Color Kit.                                                                                                                                                                                                                                                                                                
•  I understand it is only due to the current COVID-19 business closure that this Quarantine Color Kit has been made available to me.                                                                                                                                               
• I understand that this is my currently recorded formula. I understand that any further alterations or corrections will be provided at my own expense.                                                                                                                               
• I have had the process and possible results adequately explained to me. I have been given the opportunity to make requests and ask questions. I will use the written instructions provided to me and acknowledge I have access to video instructions.                                                                                                                                       
• I understand there are risks in using hair color including but, not limited to: allergy, irritation, staining- color stains everything, interaction with some metal jewelry and eyewear.
