I agree to rent a Stand Up Paddle package from Mountain Surf Paddle Sports LLC at 115 King Ave Apt B, Fayetteville WV, 25840.
I understand that I am responsible for the return of this equipment in the same state in which I received it. I agree to pay in full for any repairs or replacement if the equipment is damaged or lost. Late returns will be charged at $15 an hour or $70 per day.
I agree to repair or replace any equipment that is lost, damaged, or stolen while I am renting it, in a manner acceptable to Mountain Surf Paddle Sports LLC, Mountain Surf Paddle Sports, Mtn Surf PS at 115 King Ave Apt B, Fayetteville WV, 25840. If I fail to return such equipment in an acceptable manner, I authorize my credit card to be charged the cost incurred in repairing or replacing the equipment. Value of equipment rented: Red Paddle Co. SUP board: $900, Werner Paddle: $150, Personal flotation device: $80, SUP fin: $20, SUP hand pump: $60.
I understand that Mountain Surf Paddle Sports LLC at 115 King Ave Apt B, Fayetteville WV, 25840 has supplied me or given me the option of a personal floatation device and they have instructed me to wear it on the water.
I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS AGREEMENT BY READING BEFORE I SIGNED.
Credit Card information is needed in the case boards are not returned or damaged on return. If you are not comfortable leaving Credit Card information online, please have a credit card on you when you come to pick up your board so we can collect this information at the time of rental.
