By checking this box, I acknowledge that it is not reasonably possible for the representatives and employees of the Detroit Ink Spot to determine the possibility of an allergic reaction to the pigments or process used in my tattoo application. Furthermore, I agree to accept the risk that such a reaction is possible * Please check it here.

By checking this box, I acknowledge that it is my (the client’s) responsibility to take care of my tattoo or peircing. I have been given instructions on how to care for my tattoo or piercing. I MUST FOLLOW THE INSTRUCTIONS or my tattoo or piercing will be subject to complications with the healing process, i.e., not healing like it should. * Please check it here.

By checking this box, I acknowledge and realize that variations in color and design may exist between the tattoo selected by me, and as applied on my body. I understand that if my skin pigmentation is a deeper color tone, the colors may not appear as bright as they might be on a more fair pigmented skin tone.
