I do hereby give my permission for my participation in yoga classes through Namastay 18 Forever at Space 15 Twenty in Los Angeles, California. I further authorize, without my approval, the rendering of any medical treatment that may become necessary due to my participation in this class.
By signing this waiver, I acknowledge that this is a physically intense yoga class, am in good health and able to participate. I have visited a licensed physician within the past 6 months and have been cleared to practice strength based yoga. I acknowledge that it is my responsibility to inform my yoga instructor of any pre-existing medical conditions or injuries that may impact my practice.
In consideration of the permission granted to me to participate in yoga classes, I do hereby agree, on my own behalf, to release Namasdrake and Namastay 18 Forever, its founders, administrators, employees, representatives, and volunteers, from any and all actions, causes of action, damages, claims, or demands of whatever kind of nature which I may have for injuries, known or unknown, which are incurred by, arise from, or in any way relate to my participation in this yoga class.
I realize that I am participating in Namastay 18 Forever at my own risk. My signature is binding to this liability waiver from this day forth. I have read this release and fully understand the terms. I execute the release voluntarily and with full knowledge of its significance and consequences.
I give my permission to Namasdrake and Namastay 18 Forever and any media in attendance to publish any photographs or video taken during this class to be published on any and all forms of media, worldwide.
I hereby agree that I will not bring or consent to others bringing claim against Namastay 18 Forever on the grounds that anything contained in the property, or in the advertising and publicity used in connection herewith is defamatory, reflects adversely on me, violates any other rights whatsoever, including, without limitations, rights of privacy and publicity. I hereby release Namasdrake and Namastay 18 Forever, its creators, employees, and volunteers and assigns from and against any and all claims, demands, causes and actions, suits, costs, expenses, liabilities, and damages whatsoever that I may hereafter have against Namasdrake or Namastay 18 Forever.
Namasdrake and Namastay 18 Forever shall have the right to assign its rights hereunder, without my consent, in whole or in part, to any person, film, or corporation.
